[Gameplay] Pool party zac passive failure

I haven't figured out how to recreate this because I've only seen it once. I was playing pool party zac and the first time I died that game my passive didn't revive me like it should have, I just died. I died behind the enemy blue buff, while playing on the red team.

Sorry I don't have a lot of information, but again I only saw it once(in two games since buying the skin).
