Aatrox Revival Bug

There has been many a game where i have been fighting as Aatrox with a full Blood Well fully knowing i will revive with my passive and give my team enough time to follow up or for my cd's to come up. But, when i die i don't revive. I know my passive hasn't been down because i check that every time before i even think about going in on a risky fight. This bug has caused me to lose a lot of ranked games recently so for now i can only play normals, where it is still happening on a regular basis. Any insight on this issue would be very welcome, and lmk if it has happened to you. Thank you.
