Fiora Vs Fiora Passive not working

I don't have any proof, a replay or any way to recreate this but in a Fiora vs Fiora top lane, dueling the enemy and hitting a vital would cause no more vitals to spawn. The only way to fix it was to walk out of range and reset the passive
