riot still has not fixed Vi, and now with this patch she has another bug. Flash q no longer works, dying while charging Q disables Q until you restart client, dying while ulting also disables ult until you restart client

For the dieing while charging Q and the ulting one try turning smart cast on/off, that should fix it. Sounds like the same bug that sion has/had.
The Q-Flash thing may not be a bug, sounds like most of the charge-flash combos are disabled.
