Rare Katarina Bug

Hello Rito , There is a bug occuring with Katarina. The bug is that there are occasion when katarina respawns after dying and some of her abilities dont work. the Most commonly affected i believe are her q and Ultimate. The Bug also can affect her other abilities however there is a way to remove this she has to die and respawn again for her abilities to be back to normal. However this bug may occur after death and may occur permanently in a game. I am unsure if it happens to other champion although i can account for is varus's Q it has had a similar bug which has occured to me and others who have played varus
