TFT Spatula Item Bug

Usually when you combine a spatula with an item to make a character a certain class the item then falls off. EX Spatula + BF Sword on a Pyke would have the item go back to your pool as he is already an assassin. Well the last game I played i mixed the items required to make a champ glacial. I did it on Ashe yet the item didn't go back to my item pool. I even ended up evolving her and the item stayed on her. Sadly I don't have a screenshot as the button to take one is F12 and not Print Screen. While I'm not sure what lead this to happen I do think it should be noted that the ashe in question was on my bench instead of the field. I did have an ashe on the field but the one on the bench came from a carousel with a spatula.
