Sterak's Gage is broken after one use!??!

I had a match with Sterak's Gage vs Irelia/Kayn/Anivia. It worked one time. It stopped working and didn't activate on me, Kayn or Irelia. Here is a summary of the match with timestamps.

19:48 I was watching the fight slow. Sterak's didn't even start to stack. Right after I respawn I snatch a kill 21:50 for Kha zix. Sterak's is up against Anivia. It is not stacking. 22:19 Now I'm against Irelia, Steraks still hasn't activate. Have 900 HP, get bursted all 900. Steraks doesn't activate. Die. 24:18 1540 HP Steraks still up. Kayn bursted me down in one hit. Steraks doesn't activate. 25:45 2187 HP, 0 HP 25:50 Steraks doesn't activate. 28:18 869 HP Bursted. Sterak's doesn't activate.

I sent a replay file in a email. This is one big bug or Sterak's is just extremely useless.
