Katarina QWER stop working

There are times when Katarina Q and E stop working in the middle of a battle.

It usually happens only after level 13 and i haven't figured out which item builds it is linked to yet.

Also I can use her Q and E once but they do not reset until after she dies.

I have only seen this bug twice and have trouble recreating it, but i will try to edit this post once i have it more narrowed down.
