Not Getting Mastery Tokens

My friend has been playing Lucian for quite a long time now and since the MT system came out, he hasn't been receiving any tokens. He's also been getting S- which is the minimum rank to get Mastery 6 token to upgrade it, yet not getting any message saying he has received a token for him.

Is this a bug with his account or just the system itself?

What game mode is he playing? (only some of the normal game modes allow tokens to be earned) Also, has he been punished by the tribunal lately? (ban, chat restriction, etc)
