Super Galaxy Elise [crash]

When in human form, using the following combo on a champion or blue buff is causing my game to crash: Q, E, W, R, Q, W. A couple times today it even crashed once her E landed on a champion. I just purchased this skin today and already have been disconnected multiple times today causing my rank to continue to plummet. I am running a full repair on the client to see if that changes things. Two .tmp files were created but I am not sure how to give those to you. here are some screenshots if it means anything
