Player and Ping muting isn't working properly

It's been about a year that "/mute all" hasn't been working properly. Even individual mutes let people talk sometimes. It's not every game this happens. Even ping mutes are still letting me see player pings. And it's not every player that I can see their text, it's just some people. No, I understand that individual muting someone, and then "/mute all" will unmute that person. I'm not talking about that. I'm saying it legit is allowing muted people through.
