[GAMEPLAY] Disappearing Icon and Health Bar

I had two separate bugs in this match while playing Illaoi. Unfortunately, I have not figured out how to take snapshots in game yet to show either of these, so I will do my best to be descriptive.

1. At the start of the game, just to the left of my team's 4 summoner icons was an odd floating 0. Half-way through the game, just after reviving, the spot suddenly filled with my own summoner icon with the 0 sitting right where my character level would be. The icon and number just as quickly disappeared for the rest of the match.

2. I was playing against Nautilus most of the game. At some point near the very beginning of the match, he hit me with Riptide. From that point onward, my health bar (the one at the bottom of the screen, not the one above my head) would only ever go DOWN, not up. The numbers displayed correctly (so it would show 876 / 1000 HP numerically), but after my subsequent death, the bar stayed black and empty the entire rest of the game. Every time I healed or took damage, the bar would flash red as if I were losing health.

Extra Details: Match played on patch 6.22, Windows 10 PC, the old client (I have not updated to te beta client)
