I was in a game a few days ago with a LeBlanc on my team and I as Soraka. After a teamfight LeBlanc was in such low health that the arrow under Soraka turned on. LeBlanc used her clone ability and as I went to heal her it turned out to be the clone, even though the arrow pointed toward it.

Pay attention to the colours of the clones. I don't even pay attention to the arrow, I'm too busy actually looking at what's going on with my teammates.
That said, I've healed Shaco/LB clones before to fool the enemy. It's worked too.
Arrow does point to clones. I use this to get into fights sometimes, so I don't want it removed... but I accidentally heal clones all the time XP
