Syndra and Ivern's Daisy
Syndra grabbed Daisy and took her outside of leash range. She threw Daisy, who then reappeared at Ivern's side, but dealt damage. Syndra was two screens away, and could not have thrown him there otherwise.

It is probably a bug. What I think happened is this: because she took Daisy outside of leash range, when Syndra thew her and Daisy became targetable again, Daisy reappeared at Ivern's side before she hit the ground. So when Daisy appeared at his side it still dealt the damage from Syndra's W.
Does that makes sense?
I hope I explained it well.
