Cannot Ban Force Dodge Twice in One Day

Twice today I dodged because I could not ban. At about 10 seconds left of the banning, once I decided who I wanted to ban I could click any champion, but not ban. It was greyed out as if I had not selected. Why does it happen? And why am I punished for something like that happening to me?

I have no clue on how to replicate it. I am on Mac.
