Malzahar's Malefic Visions Not jumping

I just played a match on ARAM where if I killed Karthas (I believe Pentakill skin) with Malefic Vision, it wouldn't jump to anyone, I had it happen two or three times, usually while standing next to Lux, Karma, or surrounded by minions. Has anyone else seen this happen? I didn't get a screen shot.

I also would like to ask if it will jump from Sion when he dies, normally when he's killed I'm so busy running from him I don't get a chance to notice if it did or not.
