Orbs - icon bug

Anyone else get this?

Got it on update day before this one.. League of Legends 4.19.14_11_05_21_19 - Not sure it still shows up. Was a random bug I only saw after people died 2% of the time.. so 2 out of 100 deaths.

Captured on 11/3/14 11:27 AM North American server on Mac client. I think it was in normals, but could have been ranked.
