Issues with match history

I've been playing league for a while today and played quite a few matches. It seemed fine at first but then I couldn't open March history, when I finally looked it only showed four matches I played instead of the six I had, I know because it showed only four as sion today yet I played at least one with teemo and it never showed up. So I don't know if it's a bug or what but I just thought that was odd. I've also noticed in game, occasionally active effects placed on my character would show up as a blue square above the ability bar instead of the actual ability's icon
