Game Freezes at End Of Match [CRASH]

At the end of about 50% of my games since the new patch, my game freezes as soon at the Enemy Nexus is destroyed. The only solution I have found for this is to restart my whole computer, as tabbing out or trying to bring up the task manager simply exits the game and brings up a black screen and the tool bar at the bottom.

Is this a common problem with the new patch, or has my game data gone screwy?
