During a game, my friend played Tahm Kench and sometimes when he ulted and I stepped inside of him to teleport with him, the ability would revert to his W ability. As in he would walk around with me in his belly rather than teleport us, without pressing any additional buttons after he pressed the ult.
Yeah iv been getting this bug as well, The ult would say it went off and is on CD but it turns into a W even though the W is not on CD, But if 
ults by himself it goes off without a hitch, I have tried the ult with normal settings (with out quick cast) it seems to work more like that if someone comes with me in the ult but it still happens time to time. GG
