Mordekaiser ult not giving the ghost on kill / Ghost command issues

Recently myself and others have been playing mordekaiser and I have used the ult, it goes on cooldown, I clearly see the ghost flying above the targets head and when they die (2-3 seconds after the ult lands on them) they do not become a ghost for morde. There was no evidence of QSS/merc scim.

This has happened twice in the last 20 games so it is not as rare as it should be.

Morde although thoroughly destroyed from the rework is still a fun champ to play. But when the dragon/ghost does not follow seperate commands to mordekaiser it can be quite painful to play with. Fixing his gameplay bugs up is vital to making him a healthy champion to play.
