Yorick ult bug (GAMEPLAY)

Was in ranked and was playing the first game of the day as Yorick About halfway through the game I was top fighting 2 people when i ulted myself and died, coming back to life and killing one of them. I then cleared the lane and waited to die, and then didn't. I B'd and then went mid to see what would happen if i died again. Well, I died to a 1v4 but instantly popped back to base and with no death counter increase from 1 to 2. I then went mid again and died for real, with the bug not repeating the rest of the match, which included me ulting others and myself with no other strange occurences. This happened on 8/20/2015.

That happened to me today! I was playing Kalista and my teams Yorick ulted me when I died so i revived and did that permanently and when Orianna and Fizz almost killed me (I was supposed to die), I instantly teleported back to base without a death recorded.
