Teamsize item didn't increase team size

I created the team size increasing item (spatula^2) while in combat, and so it basically became null. I couldn't add another champ after the match. Was Lv5 with 5 champs. Tried swapping the champ with item out, even selling him and re-equipping to another champ; still nothing.
