I know it isn't important, but with new items coming out or being reworked, older items look extremely different and, just like splash art, can be dated or a completely other art style (examples are:

For the observant, and I mean really observant, because I never saw this until it was pointed out to me, that crest belongs to either the Lightshield Dynasty or the current King of Demacia, making Randuin's Omen a demacian shield. What's important to note though is that it was given a new icon because it's part of a big item overhaul coming next patch for armour and mr items. The icon update was something they could add there as there was all ready a mechanical update for the item going on.
So yes, they'll update item icons, it'll just depend on when priorities work out. Black Cleaver and Zeke's were changed specifically because those items underwent big changes. Randuin's is getting some pretty major changes to so it worked out. Might be coming this patch, we'll have to see.

Try downloading the image and inlarging it further, you will see a shield crest the same as the Lightshield crest. At first I thought it had something to do with Leona, but after researching the Demacian Lightshield crest for wiki reasons, I am very certain that this item is Demacian in origins.

It's not as clear as Randuin's; more like an engraving than an emblem, but yeah, looks like the crest. I guess Aegis really is Demacian. Now I feel bad because I saw that on the wiki and was like "dafuq?" and removed it. I'm a dunce. slams head in to desk
