Yes, it's kinda easy:

Invent a time machine and travel back to the time where team builder did still exist. Then queue up 5 times as assassin.

After playing this games, the icon will appear in your summoner icons.

You can get it as a rare random drop from crates I think, but other than that, I fear that you have come about ten months too late, good sir.

I am pissed because i played at least 6 games with Zed and didn't get any icons...
