It's the Madden of the Mist, his ult.

But does anyone know anything about like their relationship or like who she was before? or was she just created when the isles got corrupted?

She isn't really a she; it's an amalgamation of souls just like everything else in the mists. Effectively it's the "voice" of the mists screwing with Yorick.

The maiden of the mist, also known has the black mist. You can see it around him by his cape, which the only thing in it's way on fully consuming him is the vial around his neck.

His ultimate, which is a bunch of souls all clumped into a ball of black mist. Other than that, not much is known about it other than it hates Yorick's persistence.
