Personally, I think Mundo and Evelynn are in the most dire need of new icons. I'm really hoping they're in the next batch of new ability icons Riot sends out (which I hope comes soon, they're not on the same scale as a champion splash but I do understand they take time).
There were these new icons for Janna's abilities which I don't know if Riot did them or if it was fan made, but they were really good.

I'm surprised Veigar didn't get new ability icons during the mage update. They're not the worst; however, they could look better.
Zed, and have the W a little more clear when he's out of range of his shadow
Quinn's. They're not terribly old but I do think they could be adapted to the new style.
