Aren't you supposed to get a little message when a player is punished and you helped by reporting him? (On my other account btw)
This fuckin guy was afk for the first 5-10 minutes of the game and when he came back he repeatedly ran down mid/bot/top and died. Riot, don't you think when a player goes 0/15/0 or around there they should just be perma banned on the spot?

And perma banning someone innocent is really uncalled for.
I think the notifications are only for chat related punishments at present. 0/15/0 is not necessarily unheard of. Sometimes you get countered in every lane, but the enemy doesn't know how to close the game. It happens.
On another note, my understanding is that only the last person to report them (and cause a punishment) gets any notice.
e.g. if 10 players report someone and the 10th report causes a punishment, player 10 will get a notice about it, players 1-9 will not.
