um, no? I mean, even if it was, they'd have to report each other, but if they are all agreeing to the surrender, then that sounds like they are all together on whatever plan it is
Bit of a weird answer there: It depends on the reason.
What I know for sure is that Riot doesn't like when people intentionally screw with the ranking system, so intentionally dropping your MMR is not allowed, even though that's most likely not covered by the IFS and will require a manual review.
I also remember some time ago people got banned for exploiting some kind of event that had a fairly high reward for every game, regardless of how long it lasted or who won (really don't remember the specifics) by immediately declaring to the enemy team that they would open mid every single game.
Generally: If there are no ulterior motives behind the FF it's probably fine. If there are, it's likely going to be a case-by-case decision made by Riot after a manual review.
Uhhh... no. It's not a crime to give up, especially when 5 people agreed to it. People have their own reason for giving up in game. Maybe they're not enjoying it, maybe something came up, or maybe they simply didn't want to play.

If it's five, they clearly planned on it. It's bound to be a unanimous vote.
If every one agrees to it, no.
If it was a premade of 4 and they forced the solo guy to ff even when winning by a margin, then it is punishable.
