So today was the first time I had to leave a game in over 2 years of playing and it asked me to agree if I promise not to leave another game. My question is how many attempts do i get before I get perm-banned?

From my understanding of LeaverBuster, it's a bit of a credit or karma system.
The first few times you'll be fine. At most you'll have to wait 10 minutes before you're allowed to queue for a game. After you hit a certain number of games left in a specific span of time, LeaverBuster begins to suspend your account. The suspension period increases exponentially for every new threshold of games left.
You can fix your "credit" or "karma" by playing large amounts of games to the end. In which case, you'll return below threshold and suffer smaller punishments or go back to not being punished.
I haven't left a game since I was in highschool years ago, so I'm not completely certain how it works.
