Why was I permanently banned?

I can't even log in to the lobby screen to see chat logs, or anything else. It gives me a message saying I was permanently banned for actions preformed on my account and says an email was sent with further info. Yet all the email says is "Actions that have been performed on your account are in violation of the League of Legends Terms of Use. You can review these terms at http://www.leagueoflegends.com/legal/termsofuse "

I just want to know what happened..

This was a MANUAL Account ban, not automated. You NEVER Get automated chat logs for those unless you ask for them in a support ticket, and in some of these cases, these don't involve in-game chat that can be logged (like stuff outside of the actual in game client when a lobby launches into the game itself).

Send in a ticket. Likely the permaban is a safety measure since your account might have been compromised.

Contact support, it sounds like it's just a safety ban. They will walk you through things and help you out with your ban.

Send a ticket in to Player Support and they will be able to tell you exactly what happened. That does not sound like it was a toxicity ban, or any other sort of ban issued by the IFS as they are accompanied with a reform card. It may take support a day or so to reply to you, but they will answer your ticket and let you know what is going on. Just don't bump your ticket, and try to be patient as you wait for them to reply.

More than likely this ban was issued on your account as someone on your account was using third-party applications, programs, macros, or scripts that gave them a competitive advantage in the game or messed with critical game files. If that is the case, you should know these bans are virtually 100% accurate (and they audit them to ensure players are not wrongly banned for it), and that even if it wasn't you, if you willingly let someone else access your account you are considered responsible for their actions.
