So is there a particular reason as to why you didnt learn the first time? Once you get a 14 day ban (especially if you went through the other punishment tiers) they tell you as a Final Warning that they next punishment is a Permaban, and that anything big or small, in terms of breaking the rules would result in that. You need to be on your best behavior when getting off of a 14 day ban.
What about the other three chances you were given?
They do not reverse bans unless done in error. Your best shot would be to make a support ticket.

you won't get unbanned if you were properly punished with the permaban
