I know you can only hit the red exclamation once. But if you right click on their name a drop down menu appears that doesn't have a restriction on the number of times you can report. How many of these are considered real reports? Does it only modify the one each time instead of submitting a new report against them?

Multiple reports does the same thing as one report. It just flags the system to check that specific account for toxicity for that game. If something was found, it would accumulate a report against that account.
When there are enough accumulated reports, the system will punish that player.
