A guy just casually told me to get cancer and after i reported him he still doesn't have any penalties ...

How do you know he didn't get penalties?
He's not the only report, and you don't know if he has gotten any penalties because you get a notification if it was your report that caused the punishment.
The behavior in the community through the years has become worse, Riot's automated system catches the most flagrant offenders but somewhat more mild cases such as the one you described will rarely get punished unfortunately.
