My account, eonman30, on the US server has been perm banned. I know that what i did/said was a huge disgrace to the league of legends community and it will never happen again. Every time that i feel like flaming a team mate or being toxic in general I can remember this. You have a wonderful game and I am someone who has been spoiling it. Just give me one last chance to prove that i will not be toxic anymore.
Unfortunately you cannot "appeal" to unban your account.
Your chance to prove you are no longer toxic is to create a new account and show that you have reformed. I know it sucks, but that is your only option.
You'll be welcomed back should you choose to make a new account and start over again :) But if you choose not to that is fine. But your old account can never be unbanned.
