When you get your account permanently suspended for toxicity, it's gone. There's no getting it back, no matter how much you can or have changed since then. The 14-day ban is the final warning, and you chose not to listen to it.
The forum is not the best place to make this request given you're unlikely to get a Riot response here. You can attempt to contact Riot through a support ticket but getting your account back if the ban was legitimate is highly unprecedented to the point of it being almost unheard of.
The definition of permanent is remaining unchanged indefinitely. Meaning; Nope. It's gone.
