Permanently suspended account-- why?
I'm a little bit confused as to how or why this happened-- maybe someone could help me-- last time I played was in January of 2015.

Most likely you were sent an Email at that time, detailing why you were banned. It could be in your spam filter if you can't find it.
Also, try googling the date until which you are banned for, that is actually a ban code and can give insight into the ban cause.
Submit a ticket or check your email. Either a program you used or you got hacked or something.
If it's not your fault RIOT will sort it out.
