How is being told to die in a car accident not a bannedable offense?

Telling someone to die is definitely a bannable offense, but there are other factors with the report system to take into account.
There are certain things that can and will result in an escalated punishment (14 day or permanent ban) even if done in only on game, and I'd say that death threats/telling someone that they should die is definitely on that list. However, an account won't receive a punishment unless they are reviewed by the Instant Feedback System, and the thing is, not every report will put an account up for review. An account needs to reach a certain threshold of reported games before being put up for review by the system.
So, if this person wasn't banned after your report, it's possible that they simply haven't reached the threshold of reported games needed to be reviewed by the system. This doesn't mean that their behavior is acceptable, because something like that isn't, nor does it mean that your report was pointless, as it will definitely help contribute to them getting punished if they continue to behave like that.
As a side note, you being the only one who could report this player doesn't really matter. The system doesn't go by the raw number of reports a player receives, it goes by the number of games they are reported in, and it only takes a single report to flag a game. Any additional reports don't really do anything extra. This essentially means that 9 reports in a single game is treated the same as 1 report, as the game will look at it as a single reported game. So even if you were the only person to report someone, your 1 report is enough to flag that game.

how is intent feeding 7 games in a row not perma ban worthy instantly? why does it take weeks to ban people actualy ruining the game over taking minutes to ban someone who told his mid laner to stop fucking suiciding ?

I got Perma Banned a while back. I was arguing with my ADC because she kept nagging the hell out of me saying zyra mid wasnt viable and that I was trash. I didn't say anything harsh back, Just simply debating back that it is viable and she should be quiet. However the whole team was in a party together except me. We reported each other, she didn't get banned, even with all her death threats and stupid comments, yet i got perma banned purely for being the one with more reports
