When you login, you are displayed with a reform card with the exact reason and evidence. Happy to paste the logs here if you'd like.
Why did it result in a perma ban? Because when you got your 14-day ban last month for the same reason, you were told that the next penalty would be a perma-ban.
If you didn't get an e-mail from Riot and your client isn't telling you, send in a ticket to Riot Support. They'll help you figure it out.
