Never. The champions I play In URF are not popular or beyond broken to ever be considered banned. I'm talking about

Never, actually. The one time they banned the champ I wanted I had forgotten to actually declare the champion.
One time - some dude was hella mad because I banned Fizz and took “revenge“ by doing the same for my champion. He didn't declare Fizz at all and didn't write a single thing in the chat regarding this issue - seems like I should read minds to know what he plans to play.
My declared pick banned by my teammates? Never, actually.
I gave it away once though, because that person asked really nicely.

I ban him but I ban him in general so idk if that counts.

None. I never declare and never ban. Never a problem for me.

None. For some reason no one thinks to ban Leona when she is arguably the most fustrating champion in urf to play against. Especially If the Leona went undying grasp and full HP build.
I have seen teammates ban eachothers champs not because they don't want to face it. But because they are trying to force their team to play specific op unfun champs.
Accidentally banned someone's Fizz because I just auto-ban him when I lock in URF. I dodged the game after apologizing.
