Just recently my account had been permanently banned for third-party softwares. This had happened because my account had been hacked by someone and this person started playing ranked games and doing well in them. I had never previously played a ranked game before so I have no reason to be boosted. I also have evidence of a chat log from my friend of him talking to the hacker.

Send a ticket to the support, just be prepared to answer a lot of questions regarding your account, as they will require them as a proof that you are indeed the owner of this profile.
The problem with this is that it is almost impossible for riot to give your account back here. If cheaters find out that you can get your account back like this they will do the following things before cheating.
I really hope your account gets given back to you(and will even upvote to give this post more visibility), but don't be surprised if it wont.
How did the hacker gain your information, most hackers dont just stumble and accidentally find a new account to hack on. It's likely that you were trying to use some third-party software that was promised to work from some hacking forum but turns out it was an account grabber.
Riot will tell you that "Security of your account's credential falls to you" and that's a reason to ban you.
Did you use ANY 3rd party software that required you to fill your account details? Or even worse, did you give your shit to anyone?
