They try that twice in the form of a 10 and 25 game chat restriction. If it doesn't work, that is when they move on to an account ban. Only in the most severe cases where you need the point drilled in immediately or don't deserve a second chance will they skip the chat restrictions and do bans.
If account bans are too harsh, you should improve your behavior before it gets that far.
Depends on the offense and how often you commit it. Theres a reason you typically start with chat restrictions and work your way up to a perma.

I wonder if Riot saves the IPs of banned accounts. So that if they just continue flaming and so on, they can see if there have been similar problems from that IP before and administer punishments quicker. Nowadays every second player has a smurf, if you play plat on one account it's not too hard to get back to that level with a smurf. If one starts talking ugly again off that IP, 14 day suspension right off the bat.
