Completely unfair that I get back from my vacation only to be able to play 3 games, in which the one before I was UNRIGHTFULLY banned, I had told a player that he was going to be reported for being toxic and threatening to afk. OK I would COMPLETELY understand a warning because of the circumstances and such, BUT TO BE BANNED FOR A 4 WEEK OLD CHAT LOG IN UNACCEPTABLE RIOT.

-10-game chat restriction. -25-game chat restriction. -2-week ban. -Permanent ban.
Whenever you are punished, you go to the next highest available tier/level, which in your case I'm assuming is a 2-week ban.
If you post your Reform Card then we can try to figure out what exactly got you banned in the first place here.

TL;DR - I shouldn't be punished because I'm guilty, the evidence is old!
I think the 3 month window is roughly based on how many games it takes to demonstrate yes, you've improved your behavior if you play regularly. I'm not entirely sure if there's currently a game count factored in, or not.
I'd be surprised if there's not, otherwise it would be possible to dodge increasing punishment tiers by cycling accounts.

Not sure if this was said already, but time will not erase your report record. You must play more to dilute the negative record with positive games. One month is not even that much time; do not expect that you'll be forgiven for toxic behavior just because you went on vacation.
