Has Riot ever IP banned anyone?

IP bans are easy to get around with proxy / DNS changes.
IP bans also risk banning a bunch of innocent people in same houses / public spaces such as schools / internet cafes / libraries / McDonalds and Starbucks etc.

Nope, they can't, that's why many people have a second account and "banned" players keep playing.
I've been banned twice.
They did once a couple of years ago. However, they quickly changed that because upon further inspection the IP ban actually stopped an entire college campus from being able to play the game. Because of that and because IP bans are fairly easy to get around Riot doesn't do those kinds of bans anymore.
