Player keeps on dodging in champ select

I'm quite new to League, but I do know that there are players who dodge. This one particular player (won't name-shame and can't because I don't remember) has been in a champ select with me twice, locked in, and then dodged or just not locked in at all, but dodged nonetheless. Is there a way to see who you queued up with recently (such as who was in a Champ select), or would it be better to submit a support ticket about this. With that being said, it could be that something came up twice in a row, even if he did lock in; however, it does seem highly unlikely.

I'll most likely send the support ticket anyways, but any additional info is nice. Thanks in advance.
