Is there no way to view a history of punishments / "reforms" / "time-til-next-reform" on the history of my account? It sure seems as though this information would be easy to provide the end user, what with the entire IFS system being automated and all.

It's an upsetting event most people tend not to forget. If you make a habit of continuously getting punished over multiple accounts, then I can see forgetting your punishment history is a thing.
As for where your "reformed" toxic meter go, I can see the reason Rito will not give it to you so people won't try to play with the meter and be as toxic as they want until they are near that punishment line.

That's beside the point. I think a "formal history" of one's punishments and reforms should be readily available. The entire system is automated and all of this stuff is logged and recorded.

As far as your punishment historty, the premise is that you should know or remember what punishment you've received. if you don't remember it, you can also ask support. It is not something that needs to be accessible in the client or website. Submit support ticket if you really want it.
Rito is a data hogger, they logged and recorded god know how much data. Not all needs to be readily available on the front end, but most can be if you ask support.

It doesn't give you a meter as far as how close you are to being scott-free, but it does tell you how much you've been reported in the last 3 months. Which would be helpful information to know. This can be found under the Riot Games Privacy Webpage. The first page is quite basic information regarding your account. It has a button to click if you are looking for more information. Clicking it will bring you to page 2, which gives detailed instructions on how to fill out a request and submit it to Riot.
Step 1: Write a letter to Riot. It must be hand-written and include the following information:
Full name E-mail (to receive the information) Date of the request Reason for the request
In order to further verify ownership of the account, please also include:
Summoner Name User Name Current Region Signup Region Date of Birth Original e-mail address used to register the account Creation date of the account (month and year) Location where account was registered (city and country) Current IP Address Last time you had access to the account (month/day/year) First purchase made with RP First 5 champions purchased Name of any refunded champions or skins Earliest gifted content Has your e-mail account ever been compromised? If so, which e-mail and when? List of alt/smurf accounts (if applicable)
Step 2: Please send a $5 payment to Riot Games via PayPal at one of the following addresses and attach the receipt to the written letter:

I tried to salvage the formatting from the original post, on that page, but it was not easy. It's best just to click there and log in for yourself. While it won't tell you whether you are teetering on the line of a punishment, it will give you a good idea of whether you're being reported ALOT or if your number of reports has gone down significantly. Yes, it does have a minimal cost, but that is associated with the high level of work that Support has to do to actually compile all this information for you after verifying your identity.
I've been tempted many, many times to do this for myself, just to see whether or not I've been being a naughty little support and not realizing it. I never used to know the site existed until one of my friends told me that I had to look at it and find out just how much RP I'd bought over the years.... And there became a very strict, totalitarian sense of anti-toxic self-control. It's like a guardian angel that sits on your shoulder, only she's a drill sergeant who barks the number of dollars I've spent anytime I'm tempted to snap back at some nitwit.

They won't tell you when your punishment tier decreases or when you're nearing the next punishment for the reasons zPOOPz and Jo0o mentioned.

I can assure you, a week after this would be available, there would be a website called: lol-punishment-estimator.com or so, and you'd enter your reports/punishments, and it will calculate and estimate how many times it would take until you get your next chat restriction and/or ban.
so no, I would not like that to be information easily obtainable.
