There should be an automatic system, when someone is inting, and afking and trolling.
Lets say, one dude gets like 5 reports on those occassions, then he will be banned 1 game of ranked. 10 reports, and 2 games.
Also there should be a category for trolling. As trolling is most confused thing around. Sometimes offmeta is considered trolling, but thats not the case.
There are automated systems for these, "Intentional Feeder Detection" and "Leaver Buster"! They check for the offending behavior though, simply reporting someone can't trigger a punishment if they don't find anything wrong. However, trolling/inting is notoriously difficult to detect, because you have to prove beyond a reasonable doubt that the actions were intentional and not just errors in judgment/bad gameplay. Reporting the behavior helps create a case against a player to notice a pattern of it, though, so that they may be punished even when trying to avoid it.

mass reporting results in a punishment, is not a good idea as it would make trolls just report to get people punished for nothing(imo) like ohh I'll say xyz flamed me, enemy team will report him with me, xyz is now punished for doing nothing
