I just got perma banned and my chatlogs dont reflect a perma ban (IMO)

I have been playing since Pre-Season 1 and just got one of my many accounts Permanently Banned. Attached is the chatlog to my one and only game of chatlog provided. Thoughts? Is this perma ban deserved? I don't think it is, but if it happened, then perhaps the public can help me see why this is perma ban worthy. I do appreciate constructive feedback, as I have 4 more accounts to play on I'd like to avoid losing them as well, but I really do not see this as a perma ban. I understand how the banning works in that I have received prior punishments, but if this is all it took to get the final nail in the coffin - then idk... seems a little odd to me. I could be wrong though!

EDIT: I wanted to make a quick edit that since Riot Tantram advised me to open a ticket, I will do that. I never like leaving people in the dark so when the ticket is responded to, and regardless of how it is resolved. I will make a final edit letting you all know the outcome.

FINAL EDIT: I made a final posting as promised to allow those who helped me see the outcome. Thank you all for the constructive feedback. I'll try it Riot's way and see if they can handle all my bullies for me. Admittedly it has been a long time since I've asked an adult to handle bullies, but it worked in elementary I can only assume it will work again.
