There are indeed 4 tiers of punishment;
- 10 game chat restriction
- 25 game chat restriction
- 14 day account ban
- Permanent account ban
You only skip tiers when it's warranted due to extreme toxic behavior. If you believe you skipped tiers by error/unfairly, I recommend posting your reform card(s) and telling us about any prior punishments. If you want to contest the ban you have to submit a ticket, but Riot will typically not reduce/remove any punishment that was not in error.
That's usually the case already.
