i addressed a similar issue in my post an hour ago, in short, players (especially low elo) are toxic 8/10 times (at least in euw, but was 3/10 toxic players in na) and the sheer amount of toxic players make u think, is there even a punishment system? ( i know there is)
A straw-man argument in support of a garbage automated ban system getting up-voted? Move along, nothing to see.
To be fair, I think the report feedback system doesn't really work. I've reported around two dozen people for telling me or a teammate to kys, which I've seen is apparently a 0 tolerance phrase, and rightfully so, yet I have not once gotten a message indicating they were punished. And unless I'm mistaken, the feedback system is supposed to tell you if your report was the one that got them a punishment. So if it's 0 tolerance, and I reported them, I should definitely get a feedback message, but don't.

i remember i had level 5 honor, i inted one game becuase i was being trolled and i got banned 14 days. the other players no punishment against them lol.
just recently ive been reporting players that admit to griefing/inting/ruining the game in a not so hehe xd way and still nothing happens.
I wonder if i admitted to ruining a game if i would get perm banned immediately after the game or not.
